Imandix Cover Pro 0.9.0.3 (Google it) or download this full version

Select single cover presentation
Browse your cover and select create presentation
Select cover type Blu-Ray Viva case
Unchecked the Show layer and Show disk
Use colors 242x242x242 in both (Back and Bottom)
Unchecked the Fade
Finally go to export tab and put cover dimension 765 x 574
Select Output format PNG and save your presentation. Here is my output
